How do I make an appointment?
To schedule an appointment at Elite Rehab and Sports Medicine please call us at (660) 956-9925. Your physician will write a prescription for physical therapy treatment based on your condition. Physicians may fax prescriptions for physical therapy to us at (660) 956-9924. We are available to answer your questions during our office hours of 7 am- 5 pm.
Where are you located?
Our clinic is located inside Warehouse 660 at 616 E. Shepherd Ave. in Kirksville, MO. You can access our office at the North entrance to the Warehouse.
How can I join warehouse 660?
